This form is designed to save you time, as once you fill it out, you can simply submit it and you will taken to a page with the order form pre-filled for mailing or faxing to Ovusoft, LLC.  After submitting this information by clicking the Build Pre-Printed Form button, please print it from your browser and then mail or fax it to us.  The mailing address and fax number are included on the final pre-printed document.

If you are paying by credit card via fax or mail, note that there is no line on this form to fill in your credit card number.  However, there is a place on the printed form for you to manually record your credit card number, your expiration date, and your signature.

TCOYF System Requirements
A PC running Microsoft Windows 95/98/ME or NT/2000/XP, with a minimum of 64 MB of memory (128 MB if running Windows XP) and 30 MB of free disk space.

TCOYF also requires that you have Microsoft Internet Explorer 4.0 or later and Microsoft Data Access Components ("MDAC") installed on your computer.  Both of these items are included on the CD-ROM (but not in the download version).

Windows ME, 2000 and XP come pre-loaded with IE and MDAC, earlier versions of Windows may require them to be downloaded and installed separately.  If you have trouble or are in doubt, you should order the CD-ROM.
